രക്തം ലയിപ്പിക്കുന്ന മരുന്നുകൾ ( Warfarin/ Acitrom ) കഴിക്കുന്ന രോഗികൾ
1. COVID-19 വാക്സിൻ കുത്തിവയ്പ്പിന് മുമ്പ് ഇനിപ്പറയുന്ന രക്തപരിശോധന നടത്തുക
PT-INR , Platelet counts
2. PT-INR 2-2.5ൽ കുറവാണെങ്കിൽ മാത്രമേ വാക്സിൻ എടുക്കാൻ കഴിയൂ
3. പ്ലാറ്റ്ലെറ്റിന്റെ എണ്ണം Normal- യിരിക്കണം4. PT-INR, പ്ലാറ്റ്ലെറ്റിന്റെ എണ്ണം എന്നിവയുടെ അസാധാരണ ഫലങ്ങൾ ( Abnormal Results) ഉണ്ടെങ്കിൽ വാക്സിനേഷന് മുമ്പ് നിങ്ങൾ വൈദ്യസഹായം തേടണം.
5. രക്തം ലയിപ്പിക്കുന്ന മരുന്നുകൾ കഴിക്കുന്ന രോഗി വാക്സിൻ കുത്തിവച്ച ശേഷം, 5 മിനിറ്റ് cotton ഉപയോഗിച്ച് കുത്തിവച്ച സ്ഥലത്ത് കർശനമായി അമർത്തണം.6. ആസ്പിരിൻ /ക്ലോപ്പിഡോഗ്രൽ (Aspirin/ clopidogrel) എടുക്കുന്ന ആളുകൾക്ക് വാക്സിൻ കുത്തിവയ്പ് എടുക്കാം പക്ഷേ വാക്സിൻ കുത്തിവച്ച ശേഷം, 5 മിനിറ്റ് cotton ഉപയോഗിച്ച് കുത്തിവച്ച സ്ഥലത്ത് കർശനമായി അമർത്തണം.

Patient on blood thinning agents must be careful before
taking the COVI Vaccine
People on drugs like Warfarin / Acitrom) should be careful about 2 things before vaccination:
1. Check their - PT-INR, Platlet counts , Hb before
aciination. If PT-INR is less than 2-2.5 and Platelet count normal, they can take the vaccine. if not seek medical
attention before vaccination.
2. All patients on any of the above drugs, after injecting
the vaccine, they must press tightly against the injection site with cotton
for minimum 5 minutes. Donot rub, only press.
3. Patients on aspirin or clopidogrel can take the
vaccination but they should press tightly against the injection site with
cotton for minimum 5 minutes.

ReplyDeleteThere are 2 school of thoughts regarding post IM ( intramuscular) injection care.
1. First is the classical teaching of rubbing /massaging the site of injection to relieve pain and assumed to increase the absorption of injection . This is not something we learn from med school but every other person who have had a shot of injection in their life have done it, whitout muvh scientific reason, but aimed mainlu to alivate the pain.
2. Second teaching is press the site of IM INJECTION, and Not to rub it. This is often taught in nursing schools with academic standard with the scientific reasons behind it being:-
1. Rubbing the site of injection will disperse the particles to subcutaneous plain and interfere with the absorption of the particles, which was actually supposed to be absorbed from the Intra Muscular plain.
2. Rubbing /massaging can bruise the skin around due to rapid injury to small vessels, especially in some patients whobare on anticoagulants ( blood thinning agents like warfarin) or anti platelets like aspirin or pateints with collagen vascular disease / scury/ ITP.
3. For IM vaccines, we apply pressure and don't rub to avoid reactogenicity to vaccines particles. ( reactogenicity is the local or systemic reaction due to injected particles)
This are some of the crucial reason to avoid rubbing.
For a correctly done intramuscular injection procedure the aim of pressing the site of injection is to stop the bleeding by achieving hemostasis and not to Alivate pain or influence the absorption.
